We get your point Tony, but whitewater was hoping that someone would share an already-designed form for quantifying the workers' attributes or sins.
I do see many templates available online, like this

Very simple and to the point. I have never liked long, drawn out reviews. Some of my worst recollections were having to give reviews with full knowledge that there was no money for raises or incentives. Talk about deflating for both parties.

I'd caution you to proceed very carefully.
If this as not been a standard practice, stated in the employee handbook (you have that right?), and used for other employees.......with a 63 year old, you might just be contributing to your own future EEOC complaint.
Unless you plan on a stellar write-up accompanied with a raise or bonus.
Just saying.
