J America has that style - AlphaBroder and Heritage Sportswear carries them
Pennant Sportswear - like this one better

Is the puff crucial?  If not, plastisol transfers are super easy for these and I do them in house.

Actually, I wonder if puff ink works as a transfer, possibly with a quick run through the dryer after application.  Has anyone tried that before?
