Title: Does anyone have a "standard" template for a Screen Printing Contract ?

We haven't used contracts with customers in the past, but now I'm being asked for a contract.  I don't need anything complicated, just something basic that would cover whatever a screen printer would want covered and would make the client feel comfortable.
