Does anyone have a form they use for the sales people to send to the graphics department with the details or the graphics relating to an order?

1.5 man shop here, so I handle both ends. But all graphic details are part of the main order details form. Everything gets recorded: art file name (per location), location & placement, imprint dims, ink count & specific colors. It ensures I don't miss anything between sales, art processing, and production. I can't imagine not using a standardized form in a larger work environment.
